Social Security cards for you, your spouse, AND dependents
If anyone does not have a Social Security Number, you can bring the Individual Taxpayer Identification
Number (ITIN) assignment letter
An official letter from the IRS (SSA-1099 form) can be accepted in place of a Social Security card
Birth dates for everyone on the tax return
Information for all forms of income: W2s, 1099s, self-employment work, etc.
Any other tax forms: 1098s, 1095s, etc.
Calculation of income and expenses from self-employment or cash income, if applicable
Your household’s health coverage information
Health Coverage Statements: Forms 1095-A, B or C
Childcare provider information, if applicable
You will need the total amount paid to the daycare provider and the daycare provider’s tax identifying number (their Social Security number or business Employer Identification Number)
Last year’s tax return, if you have it
Savings AND checking account numbers, as well as routing number for direct deposit of refund
